Help Accessing Government Services
We are pleased to be able to help you with any questions, concerns, or issues you may have accessing provincial government services such as health care, education, family & social supports, Workplace Safety and Insurance Board (WSIB), or Ontario Works.
Congratulatory Certificates
We offer congratulatory certificates for residents celebrating a significant anniversary, birthday or other special occasion. We can also help you request a certificate from the Premier for a milestone birthday, wedding anniversary, retirement or the anniversary of your organization.

Commissioner for Taking Affidavits Services
Our office may be able to assist with commissioning certain documents free of charge for residents of Renfrew–Nipissing–Pembroke.
A Commissioner for Taking Affidavits can witness signatures on documents that require an oath, affirmation, or statutory declaration. Before signing, you must appear in person and provide valid government-issued photo identification.
We can often commission:
- Affidavits
- Statutory declarations
- Sworn statements
- Consent forms that require commissioning
- Government forms that require a Commissioner for Taking Affidavits
We cannot:
- Notarize documents
- Certify true copies of documents
- Witness wills or powers of attorney
- Provide legal advice
- Commission documents with blank sections that have not been completed
- Commission documents written in a language we cannot understand
- Commission documents if we believe the signer does not understand the document or is not signing voluntarily
Before Your Appointment
Please bring:
- The completed (but unsigned) document
- Valid government-issued photo identification
- Any instructions provided by the organization requesting the document
Important
Not all documents can be commissioned in our office. Some documents require a lawyer or Notary Public instead. If you are unsure, please contact our office in advance and we will be happy to review your document and advise whether we can assist.
